Insurance and Payment Information
We accept most insurance plans and will gladly process your claim. However, we do not accept ProviderOne and Apple Health. Please bring necessary information with you on your first visit. Insurance policies generally cover only a portion of the total treatment cost. We ask that you pay your portion of the bill at the time of treatment. If you have not paid your balance within 60 days, a rebilling fee of 1.5% will be added to your account each month until paid. We also accept Visa, MasterCard, and American Express for your convenience. Financing options are available through Care Credit.
